EMBERHEALTH CONCIERGEAll services

Ember Continuity

Someone who knows your story—or your loved one’s—every month.

Best for ongoing support

No referral fees, ever. Ember works for you and the people you choose to involve.

All support is provided remotely by phone or video. No home visits.

Who it’s for

Adults managing their own care, and family members or loved ones who need ongoing support as care changes. Requires Ember Clarity.

Your voice and choices

The person receiving support is at the center of the plan. We listen to their goals, help them express preferences and questions, and involve family members or loved ones with permission. When a representative is needed, their voice and preferences still matter and are included as fully as possible.

How our conversations work

Weekly check-ins and monthly individual or shared planning conversations are guided discussions about changes, medication or appointment questions, pending follow-ups, and responsibilities and available support. Stacey asks focused questions, tracks unanswered items, and keeps everyone working from the same plan.

What you get

  • Dedicated navigator who knows your history or that of the person you support.
  • Weekly check-in call for you, with family members or loved ones involved if you choose.
  • Monthly 60-minute planning conversation, just for you or with your chosen family members and loved ones.
  • Written weekly update for you and anyone you authorize to receive it.
  • About 10 hours of navigation a month. Extra time is $200/hour, always agreed in advance.
Appointment support, care binder, and ongoing coordination
  • Up to 4 appointments a month: prepared, joined virtually with permission for notes, and debriefed in writing.
  • Calls to providers, pharmacies, and insurers on your behalf, with authorization.
  • Living Care Binder: shared secure folder with directory, plan, and documents, updated monthly.
  • Ember Medical Bill Review included monthly.
  • Quarterly Ember Plan refresh.
  • If you or the person receiving support is hospitalized: priority response plus Hospital-to-Home support at no extra cost.
  • Same-business-day response Monday to Friday.
From a flagged concern to an answered question
  • With authorization, contact the appropriate office about conflicting instructions, missing records, unbooked referrals, or follow-up that has stalled.
  • Prepare medication and appointment questions for the treating team; record their answers and update your written plan using their confirmed instructions.
  • Track requested appointments, tests, and records: what is pending, who is responsible, and when to follow up. Clinical urgency and testing decisions belong to the treating team.
  • After supported appointments, share a written debrief with you and authorized supporters: what the clinician said, what needs clarification, and who handles each next step.
  • Help translate the agreed plan into practical arrangements: transportation questions, agency contacts, equipment supplier follow-ups, and responsibilities for you and your chosen support network. Work stays within your package’s appointment, time, and communication limits.

Timeline

Ongoing monthly support with a 3-month minimum. Weekly updates, monthly individual or shared planning conversations, and quarterly plan refreshes.

What you no longer have to do

You no longer have to track every detail alone, update loved ones separately, or rebuild the plan after each change.

What it doesn’t include

No round-the-clock availability, medical monitoring, emergency response, or hands-on care.

Illustrative, not actual clients.

Evelyn and her daughters

Evelyn’s daughters lived in different states. As appointments and support needs changed, scheduled navigation and written family updates helped them work from the same priorities.
