All support is provided remotely by phone or video. No home visits.
Who it’s for
Individuals managing complex care for themselves, and families or loved ones coordinating care for one or both parents or other loved ones, managing several specialists, or keeping trusted professionals aligned across different locations.
Your voice and choices
The person receiving support is at the center of the plan. We listen to their goals, help them express preferences and questions, and involve family members or loved ones with permission. When a representative is needed, their voice and preferences still matter and are included as fully as possible.
How our conversations work
Your check-ins and individual or shared planning conversations are guided discussions about your own care or the loved ones you support, changing concerns, unresolved care-team questions, and coordination with trusted professionals. Stacey asks focused questions and follows agreed tasks through within your written scope.
What you get
- Everything in Ember Continuity: weekly check-ins and written updates for you and authorized supporters, a monthly individual or shared planning conversation, an updated Living Care Binder, monthly Ember Medical Bill Review, and quarterly plan refreshes.
- Support for your own care or for loved ones, including both parents, with each person’s contacts, appointments, documents, and next steps organized within your agreed scope.
- Up to 8 appointments a month: prepare questions, join by phone or video with permission to take notes, and provide a written debrief.
- Evening and weekend priority access within the communication schedule agreed before starting.
- All support is provided remotely by phone or video. No home visits.
What coordination looks like
- With authorization, contact providers, pharmacies, and insurers to schedule appointments, request records, and follow up on unanswered practical questions.
- Keep a written list of open tasks: who is responsible, what is still needed, and the next follow-up.
- Coordinate with your attorney about missing planning documents and practical follow-up; legal advice stays with your attorney.
- Coordinate with your financial advisor about care-cost information and requested paperwork; financial advice stays with your advisor.
- Work with your local care manager or agencies to keep local arrangements and your plan aligned.
- Prepare your individual or shared planning agenda and share decisions, assigned responsibilities, and next steps afterward.
From a flagged concern to an answered question
- With authorization, contact the appropriate office about conflicting instructions, missing records, unbooked referrals, or follow-up that has stalled.
- Prepare medication and appointment questions for the treating team; record their answers and update your written plan using their confirmed instructions.
- Track requested appointments, tests, and records: what is pending, who is responsible, and when to follow up. Clinical urgency and testing decisions belong to the treating team.
- After supported appointments, share a written debrief with you and authorized supporters: what the clinician said, what needs clarification, and who handles each next step.
- Help translate the agreed plan into practical arrangements: transportation questions, agency contacts, equipment supplier follow-ups, and responsibilities for you and your chosen support network. Work stays within your package’s appointment, time, and communication limits.
